Ditch Maintenance Form for Works in a Wet Constructed Ditch
This Ditch Maintenance Form is referred to in the Agricultural Watercourse Maintenance Guide. It can also be found as Appendix B on page 43 of the Guide.
The form MUST be signed, with all areas completed. The completed Ditch Maintenance Form must be sent by fax or mail to DFO. Fax and mail information is provided on the Key Contacts page.
